California Healthy Kids Surveys

The California Healthy Kids Survey (CHKS) is an anonymous, confidential survey of school climate and safety, student wellness, and youth resiliency. Baldwin Park Unified School District administers the anonymous survey to students in grades five, seven, nine, and eleven. Overall, schools and the District receive generalized reports. Individual student reports are not produced. The survey data regarding local youth health risks and behaviors, school connectedness, school climate, protective factors, and school violence is reviewed and analyzed. The California Healthy Kids Survey provides insight and feedback on improving the school climate and student learning environment as the District develops the Local Control and Accountability Plan.   

Consent to Participate:

  • For fifth-grade students, Active Consent requires that a parent or legal guardian be notified in writing and give written permission for the student to participate in the survey. 
 
  • For seventh---, ninth---, and eleventh-grade students, Passive Consent requires that a parent or legal guardian be notified in writing about the survey and is allowed to review the survey. Parents must inform the school in writing if they do not want their child to participate in the survey.
