Facility Use Application

FACILITY USE REQUEST ANNOUNCEMENT

Effective June 10, 2019

 

Baldwin Park Unified School District and Facilitron are pleased to announce a new online facility request and rental system designed to provide a positive user experience for all. With the new system, reservation requests can be submitted easily and handled efficiently. Renters will be able to access photos and descriptions, see real-time availability, get estimated quotes, and pay online.

 

In partnering with Facilitron, Baldwin Park Unified School District is launching web-based facility websites for each school with facility availability – allowing facility use requests to be submitted at any time.

Although the process of requesting/approval/management of facility use is now digital, what’s implemented on Facilitron, remains the same as dictated by Baldwin Park Unified School District’s board policies on facility use. District administrative staff will make final decisions on all facility use requests. 

 

Facilitron will assist with the set-up of organization and user accounts along with verification of non-profit status (if applicable).

 

Facilitron will collect proof of insurance (Certificate of Insurance and Additional Insured Form) on Baldwin Park Unified School District’s behalf. Proof of insurance (certificate of insurance) can be conveniently uploaded into the system, and/or it can be obtained directly through Facilitron. Other documents, such as rosters, can also be uploaded on the website. 

 

Facilitron will also collect payments.  Payments can be submitted conveniently on Facilitron. Payment options include major credit cards, checks, ACH/eCheck, and Paypal.

 

The new Facilitron system is now live for all sites.  Moving forward, the only way to request use of a district facility is by creating a Facilitron account and submitting a request online. Please click HERE for an introduction to placing a request. For those who had reservations with Baldwin Park Unified School District prior to Facilitron, accounts have already been created on your behalf, and an automated email has been sent to you to create a password to access your renter account.  If you need any assistance setting up an account and/or in submitting a facility use request, you can contact Facilitron 24/7 directly at [email protected], or by calling them at: 800-272-2962 extension 1.

 

 

Be sure to submit Facility Use Application at least 10 days before your event.

Applications are accepted for the period of 1/1-6/30 and 7/1- 12/30 only and may be submitted a maximum of 2 months in advance.

 

BALDWIN PARK UNIFIED SCHOOL DISTRICT INSURANCE REQUIREMENTS

MUST ATTACH CERTIFICATE OF INSURANCE and ADDITIONAL INSURED page as proof of insurance.  The Additional Insured Page must list Baldwin Park Unified School District, 3699 N. Holly Ave, Baldwin Park 91706 as the additional insured.  

If these forms are not attached your application will be declined and you will have to resubmit with the required documents.

 

Be advised - Facility Use is NOT permitted for fields on Mondays (to allow for fields to rest). Facility use ends at 9:00 pm on weekdays and 5:00 pm on weekends. Applications that do not meet these guidelines will be declined.

 

 

**ALL ORGANIZATIONS WILL BE BILLED FOR TIME SITE IS RESERVED, NOT TIME USED**

 

 

DOES YOUR YOUTH ORGANIZATION QUALIFY AS NON-PROFIT?

 

In order to qualify as a Non-Profit Youth organization you must meet the following criteria:

 

  • You must have your 501(c) current 
  • You must have 75% of your membership or participants must be residents of the District
  • Participant rosters - must be submitted in order to be evaluated 

 

 

DOES YOUR ADULT ORGANIZATION QUALIFY FOR DIRECT COSTS?

 

In order to qualify as a Direct Cost Adult organization you must meet the following criteria:

 

  • You must have your 501(c) current 
  • You must have 75% of your membership or participants must be residents of the District
  • Participant rosters - must be submitted in order to be evaluated 

 

All information will be verified by the District

 

Proof of District residency must be provided in the form of a utility bill
